Things that jump right out at me:
1. You are standing too far from the ball. Your arms should hang down, not extend out (unless you are Moe Norman.)
2. Your baseball grip gives you too much hand action. It's fine if you can control it, but I don't think you can.
3. You have a reverse weight shift, meaning your weight moves to your left side as you take the club back.
I would suggest reading Hogan's book, and practicing some drills that address posture and proper weight-transfer. Also, your cameraman needs to do a MUCH better job of getting footage of any hot chicks that show up at the range...
