This is simply the sign of an efficient golfer. He needed to use the facilities and hit is ball right by the door to keep the unnecessary footsteps to a minimum.

If everyone would have the forethought to do this, slow play wouldn't be such an issue.

Further, everyone is making assumptions that this is a "tinkle" position, when it looks strangely like a "retching" position to me-- maybe the effect of seeing an Irish Wanky waving and winking one time too often?
