When your ball lands in the bunker but it's not plugged or anything, do you still hit behind the ball and splash it out? How about when the sand is as hard as a rock?

When the sand is hard as a rock, get your lowest bounce club because it will most effectively bury. Also, yes, almost always I hit behind the ball unless it's a fairway bunker.
